Service Dog Retrieval Study: Consent Form
The researchers request your consent for participation in a study about named retrieval errors by service dogs and service-dogs-in-training. The study requires multiple submissions to a survey over the course of a week for about twenty minutes total. This consent form asks you to allow the researcher to view your questionnaire submissions and to use your answers and comments to enhance understanding of the topic. The form also asks your permission to use related observations, images or posts as data in this study.

Participation in this study is completely voluntary. If you decide not to participate there will not be any negative consequences. Please be aware that if you decide to participate, you may stop participating at any time and you may decide not to answer any specific question.

The researcher will maintain the confidentiality of the research records or data, and all data will be destroyed after study publication. The data will be stored on a password-protected computer.

By submitting this form you are indicating that you have read the description of the study, are over the age of 18, reside in the United States, and that you agree to the terms as described.
